29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a)On and after November 20, 2012 located along low sloped roof areas of 11560 Princeton Pike Cincinnati OH 45266, 4590 Cornell Rd Blue Ash, OH 45241 and 4270 Hunt Rd Blue Ash OH 45242 commercial restaurant roof structures employees were performing extraction fan cleaning associated activities adjacent to unprotected roof sides or edges without personal protective fall arrest/restraint equipment or protected by other conventional fall protection means and were exposed to fall potentials ranging between 8.4' to 18'.
