29 CFR 1910.333(a)(1): "Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs.    a. At the worksite located at 501 West Shrock Road in Westerville, Ohio, the employer had an electrician that was replacing a fuse on a 3 phase, 480 VAC switch, but the employee did not de-energize the switch up-stream so there were no current carrying parts within the switch.

Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.    a. At the worksite located at 501 West Shrock Road in Westerville, Ohio the employer had not provided training on their Lockout Tagout program to an employee that was injured during an arc flash.

The employer shall maintain a written copy of the procedures outlined in paragraph (b)(2) and shall make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives.  Note: The written procedures may be in the form of a copy of paragraph (b) of this section.    a. At the worksite located at 501 West Shrock Road, in Westerville, Ohio the employer did not have adequate written procedures.  The Lockout / Tagout Program that the employer had did not address all items as required by the standard, such as, but not limited to, all sub-paragraphs that outline the requirements of the verification of the de-energized condition (ie. testing to determine if any energized condition exists as a result of inadvertant induced voltage or unrelated voltage backfeed).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.     a. At the worksite located at 501 West Shrock Road in Westerville, Ohio the employer had an employee that was injured from an electrical arc flash while changing a fuse on a 3 phase, 480 VAC switch, but the employee was not provided with and did not use all required PPE to protect himself from injury, such as but not limited to arc-rated clothing, arc-rated face shield or arc flash suit hood, hard-hat, safety glasses or safety goggles, and hearing protection.
