29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) At the facility, K&K Machine Shop, located in Houston, Texas, on or about 02/01/2013 - An energy control program had not been established.  Employees were exposed to caught-in hazards while performing maintenance activties such as, but not limited to, replacing blades on a MARVEL Horizontal Bandsaw (MN S330/2).

(b) In the 6750sf shop built in 1977, on or about February 01, 2013 - The AXELSON Metal Lathe lacked a guard on the chuck, exposing employees to caught-in and amputation hazards.    (c) In the 6750sf shop built in 1977, on or about February 01, 2013 - The AXELSON Metal Lathe lacked a guard on the chuck, exposing employees to caught-in and amputations hazards.
