29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    The employer does not protect employees from falling when working on a scaffold platform 2 feet or more above a point of access.  This violation was observed on or about February 1, 2013, on a 4 tier scaffold where employees exposed to a 46 foot fall hazard when climbing the crossbraces as a means of access to various tiers on a scaffold that did not have a ladder.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders are installed on scaffolds to protect employees from falling when point of access is 2 feet above or below a scaffold.

29 CFR 1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms.    The employer does not protect employees from falling when guardrails were not installed on the ends of platforms.  This violation was observed on or about February 1, 2013, on a 4 (four) tier tubular welded scaffold where employees were exposed to a 46 foot fall hazard when installing decorative stucco molding from the top tier of scaffold near ends of platforms without a fall protection system.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guardrails are installed along all open sides and ends of platforms to protect employees from fall hazards.
