29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) San Carlos Indian Reservation, Tulapai Flats Subdivision: On or about January 29, 2013, and at times prior thereto, employees were exposed to falls from heights greater than 8 feet 5 inches while erecting trusses and applying sheeting on roofs of single family housing. Fall protection was not provided.

Safety of equipment was not determined on the basis of electrical insulation:  (a) San Carlos Indian Reservation, Tulapai Flats Subdivision: On or about January 30, 2013, and at times prior thereto, the power cord connected to a Skil circular saw was missing a section of outer insulation and its inner conductors were exposed.
