29 CFR 1926.50(g): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) At the front side of the project site on or about February 5, 2012, employees mixing and applying Hansen Portland cement, were not provided with an eyewash unit/station. Per the MSDS, flush gently under running water for 15 minutes or longer, making sure that the eyelids are held open. Other than washing with water, do not attempt toremove material from eyes. Obtain medical attention for eye contact with wet cement. The first 10 to 15 seconds after exposure to a hazardous substance, are critical. Delaying treatment, even for a few seconds, may cause serious injury to employees.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. (a) At the project on or about February 5, 2013, employees mixing and applying Hansen Portland cement did not use eye and hand protection. Eye and skin contact may result in irritation to chemical burns.

29 CFR 1926.50(d)(2): The contents of the first-aid kit were not checked by the employer before being sent out on each job or at least weekly on each job to ensure that the expended items were replaced: (a) At the project on or about February 5, 2013, the employer did not inspect the Afassco #124 First Aid kit weekly, and did not ensure that expended items were replaced. The first aid kit contained only seven items.
