Model FG45T2-8 Forklift (Serial # 131846A) to lift granite, marble and limestone slabs without prior written approval by the manufacturer, on or about February 7, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Model FG45T2-8 Forklift (Serial # 131846A) to lift granite, marble and limestone slabs while the forklift was not marked to identify the attachment or the allowable weight to be lifted/moved, on or about February 7, 2013. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
