One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On top of Tram near the guide cables: On or about February 5, 2013 The mechanic was not guarded from ingoing nip-points when employees were inspecting the cables.

The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a) Tram control booth: On or about February 5, 2013 An employee used an AXIS meter which was plugged in by extension cord. The extension cord was missing the ground pin.
