29 CFR 1904.32(b)(3): A company executive did not certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete.  On or about February 05, 2013, the employer provided a copy of the OSHA 300A for the year 2012 and the annual summary was not certified by a company executive. The annual summary was certified by the Health and Safety Manager.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each annual summary is certified by a company executive.
