29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  On or about February 5, 2013, employer did not ensure that leveled walking working surfaces were provided for employees to perform their duties. There were many potholes in the parking area. This condition exposed employees to potential trip and fall hazards.
