29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  On or about February 5, 2013, employer did not ensure that a floor holes were covered. There was an open floor hole in the bus washing area and several pot holes in the parking area. This codition exposed employees to potential trip, fall and struck-by hazards.
