29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  Maintenance Shop: On February 7, 2013, and at times prior thereto, a tork timer and eletrical panels 2PNA, 2PNA1, and 2PE, which energized lighting and control panels, were blocked with storage.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  On February 7, 2013, and at times prior thereto, and in the following instances, electrical equipment was not properly guarded and live wiring was exposed;  A. Kitchen: Electrical panels 2PNK1 and 2PNK2, which energized lighting, were missing blanks.  B. Pool Pump Room: An electrical disconnect box, positioned next to panel 2PNP and energized the pool pumps, was missing proper guarding.

Kitchen: An orange extension cord was used to energize security cameras and was run through the ceiling.  B. Kitchen: An orange and two black extension cords were run through a window and used to energize a hot pot and coffee pot.   C. Boiler Room: A black extension cord was used to energize the motor on Boiler 3.
