Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to amputation hazards as a result of an unguarded foot pedal on a shear: a.) Taylor Metal Products Co. located at 700 Springmill St. Mansfield, OH 44903: On or about February 6, 2013, the employer did not assure employees were protected from amputation hazards while operating the Niagra Shear (Model#48) in that all of the foot pedals were not guarded. A feasible method of abatement, among others, would be to install a guard on the middle foot pedal with a similar type of guard in accordance with ANSI B11.4-2003 Safety Requirements for Shears E6.3.3.3 Actuating means.

Employees were exposed to fall hazards of up to 6 feet 2 inches while working around the scrap conveyor pit on mechanical power press PTC #105. b.) Taylor Metal Products, Co. located at 700 Springmill St. Mansfield, OH 44903; On or about February 6, 2013, the employer did not assure employees were protected from unguarded floor openings. Employees were exposed to fall hazards of up to 6 feet 5 inches while loading new coil and working around the coil feeding pit on mechanical power press PTC #105.

Employees were exposed to amputation hazards while loading new steel coil and oiling the steel inside the light curtains, and clearing scrap from behind the press. b.) Taylor Metal Products Co. located at 700 Springmill St. Mansfield, OH 44903: On or about 2/6/13, the employer did not assure energy control procedures were utilized on PTC #105 mechanical power press (Model # BL2-600-180X60). Employees were exposed to amputation hazards while loading new steel coil and clearing scrap inside the light curtains. c.) Taylor Metal Products Co. located at 700 Springmill St. Mansfield, OH 44903: On or about 2/6/13, the employer did not assure energy control procedures were utilized on the #51 Clearing mechanical power press (Model S2-100-60-30). Employees were exposed to amputation hazards while using the plug interlock to perform die changes. d.) Taylor Metal Products Co. located at 700 Springmill St. Mansfield, OH 44903: On or about 2/6/13, the employer did not assure energy control procedures were utilized on the Ravne #118 mecahnical power press(Model DE-315-84-48). Employees were exposed to amputation hazards while loading a new steel coil.

Taylor Metal Products Co. located at 700 Springmill St. Mansfield, OH 44903: On or about 2/6/13, the employer did not assure the PTC #105 mechanical power press (Model # BL2-600-180X60) was properly de-energized by using the established procedures. Employees were exposed to amputation hazards while loading new steel coil and clearing scrap inside the light curtains. c.) Taylor Metal Products Co. located at 700 Springmill St. Mansfield, OH 44903: On or about 2/6/13, the employer did not assure the #51 Clearing mechanical power press (Model S2-100-60-30) was properly de-energized by using the established procedures. Employees were exposed to amputation hazards while using the plug interlock to perform die changes. d.) Taylor Metal Products Co. located at 700 Springmill St. Mansfield, OH 44903; On or about February 6, 2013, the employer did not assure the means of selecting the Off, Inch, Single Stroke, and Continuous modes, respectively, was capable of being adjusted only by supervision of the employer.
