The 2x6 used as a platform extended over the scaffold support more than 12 inches and supported bricks being used.  The platform assembled and used was not designed and installed so that the cantilevered portion of the platform was able to support employees and materials without tipping and did not have guardrails that blocked employees access to the extended end, exposing the employees to a fall distance of approximately 18 feet to the ground level below.
