29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about October 30, 2014 at the worksite, employees were on an approximately 12 ft. high elevated scaffold, supplying materials to other employees working on the roof. The scaffold did not have proper guardrails nor were the employees wearing a personal fall arrest system.
