29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. a) Location: 4544 Mulhauser Rd, Hamilton, OH On or about 5 February, 2013 the employer did not ensure that employees operating forklifts received training and an evaluation on forklift operations and safety as required. Two employess currently using forklifts had not been trained as required.
