29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8m) or more above lower levels were not protected from falling by a guardrail system, safety net system, personal fall arrest system: (a) 440 East Lincoln Ave Myerstown, PA - Employees were observed performing roofing activity on a flat roof without the benefit of a fall protection system, thereby exposing employees to a fall of 9 feet 8 inches to the ground below, on or about February 7, 2013. No abatement certification or documentation required.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: (a) 440 Esat Lincoln Ave Myerstown PA - Employees were observed using a portable extension ladder that was not secured at the top to access the roof. The base of the ladder was observed on top of an unstable and unleveled soil pile, thereby exposing employees to a fall hazard of 9 feet 8 inches to the ground below, on or about February 7, 2013. No abatement certification or documentation required.
