For the Sensible Closet Company, Inc. establishment at 5844 Enterprise Parkway, Ft. Myers, FL 33905, as observed on or about 02/06/2013, employees were exposed to fire/explosion hazards associated with:    a) A poorly performing and poorly designed ventilation system that failed to capture fugitive dust from machinery such as saws and emitted the dust to the building interior resulting in dust accumulations up to  2-inches deep. The wood dust was analyzed to be a combustible, class II dust.    b) Vacuum cleaners used to collect wood dust, which was a combustible, class II dust, were not approved to be used with a class II dust, and accumulations of combustible wood dust were present in their collection bags on a daily basis.
