29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a)  Beneficial Bank, 537 Baltimore Pike, Springfield, PA- An employee performed carpentry work on plywood and planks spanning the upper flat surface of the first stage of the roof trusses, exposed to a fall hazard of approximately 20 feet above the concrete floor below, without any form of fall protection.  Condition observed on October 24, 2012.       Five Star Builders, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315816546, citation number 1, item number 1a and was affirmed as a final order on January 27, 2012, with respect to a workplace located at 210 West Chelten Avenue, Philadelphia, PA.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee exposed to fall hazards to enable them to recognize the hazards of falling and the procedures to follow in order to minimize them.  a)  Beneficial Bank, 537 Baltimore Pike, Springfield, PA- Employees were not provided training which enabled them to recognize fall hazards and to apply procedures to minimize these hazards.  An employee worked on top of a narrow platform, which was laid on top of the first stage of the roof trusses, where he was exposed to a fall hazard of approximately 20 feet without any form of fall protection.  Condition observed on October 24, 2012.   Five Star Builders, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 315816546, citation number 1, item number 1b and was affirmed as a final order on January 27, 2012, with respect to a workplace located at 210 West Chelten Avenue, Philadelphia, PA.  NOTE:  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
