Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee in that employee was exposed to crushing and struck by hazardous when off loading concrete sound barrier panels: a) On or about 02/07/13 at Route 22 South and Liberty Avenue, Hillside, NJ 07205 the employee was struck and pinned by two concrete sound barriers that fell from a flat bed trailer during an unloading process.
