29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    On or about 2/7/2013, Paulding Ave.    a) The employees were working in the side yard and there was debris obstructing their passageway.

Wooden plugs driven into holes in masonry, concrete, plaster, or similar materials shall not be used:    On or about 2/7/2013, Paulding Ave.    a) The employees used the outlet to energize their tools. It was not secured in place.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:    On or about 2/7/2013, 1st and 2nd Floors    a) The 110 electrical panels were live and not covered.

The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors:    On or about 2/7/2013, 2nd Floor and Side Yard    a) The employees were using extension cords to energize their tools. They did not have a ground fault circuit interupters.
