OSH Act of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to fall and crushing hazards: At the worksite located at 12364 Potter Cir, Omaha, NE 68142 the employer is failing to protect employees from fall and crushing hazards while using a Caterpillar TH63 all-terrain forklift all-terrain to elevate employees in a personnel platform to heights of 13 feet above the ground. The platform has an overall width of 16 feet, which extends 4 feet 9 inches beyond each side of the 6 feet 6 inches axel width of the forklift. Amongst other means, one feasible means of abatement is to ensure that the platform dimensions do not to exceed the axel base by more than 10 inches as in accordance with ANSI B56.6 2002.
