29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.    On or about Wednesday February 6, 2013 at 3029 Heritage Place Milledgeville, GA 31061;    Employees were using a circular saw to cut wood to frame the building, in which the extension cord was damaged exposing employees to approximately 120 volts.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about Wednesday February 6, 2013 at 3029 Heritage Place Milledgeville, GA 31061;    Employees standing in the unguarded wall and window openings were exposed to a fall hazard of up to 10 feet.     The Garcia Building Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 316301332, citation number 1, item number 2 and was affirmed as a final order on 8/3/2012, with respect to a workplace located at 20 Jubilee Circle, Pell City, AL 35125.
