29 CFR 1910.146(d)(3)(vi): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices to verify that conditions in a permit space were acceptable for entry throughout the duration of an authorized entry: On or about February 6, 2013, an employee of Archer Daniels Midland Co. was the entry supervisor for a permit-required confined space entry into the #2 Triple 2nd Effect Separatorto perform water blasting operations and the employer did not implement procedures and practices to verify that conditions in the space were acceptable throughout the duration of the entry. Built-up material fell from the ceiling of the space during the blasting, striking a contract employee.
