29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a) Guard railing system was not in place for employees who stand on top of pieces of equipment that had been milled  to removed cut metal material l and/or shavings exposing them to falls in excess of six(6) feet.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a) Machine guarding was not provided for the unused portion of the Johnson HA-16 horizontal bandsaw exposing employees to rotating saw blade.    b) Machine guarding was not provided on the Leblond machine lathe to prevent employees exposure to rotating parts.
