29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. At the establishment, the employer did not provide recertification of powered industrial truck operators every three years. Employees operating power industrial trucks to move materials and products throughout the warehouse were exposed to physical injuries related to power industrial trucks operations.
