29 CFR 1926.404(f)(6):     Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.    (a)  At the project on or about February 13, 2013, and at times prior thereto, the path to ground from 110 VAC receptacle was not permanent and continuous.  The receptacle was used to supply power to the 110 VAC Makita HM 1201, demolition hammer, used to chip concrete around the door jamb.

29 CFR 1926.405(g)(2)(iv):     Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    (a)  At the project on or about February 13, 2013, and at times prior thereto, the outer insulation of the 50 foot extension cord was not secured to the plug.  The cord was used to supply power to a 110 VAC Makita HM 1201, Demolition Hammer, used to chip concrete around the door jamb.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    (a)  At the front of the project site (entry door) on or about February 13, 2013, there was no portable ladders, hook-on ladders, attachable ladders, to safely access the working platform of the scaffold.  An employee was observed climbing the end frame of the scaffold to access the working platform.
