29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: At Andersen Air Force Base, outside building 18001 on November 13, 2013, a synthetic sling was damaged and was not immediately removed from service. The sling had snags and tears and was used to rig and lower shoring material into an excavation.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: At Andersen Air Force Base, outside building 18001 on November 13, 2013, and at times prior thereto, the path to ground was not permanent and continuous. A 50 foot extension cord had an "Open Ground." The cord was used in conjunction with a Makita circular saw.
