29 CFR 1910.22(b)(2): Permanent aisles and passageways were not appropriately marked: WAREHOUSE BETWEEN FREEZER AND COOLER: On or about February 13, 2013, an overhead curtain door located between the freezer and cooler intended for fork truck traffic was used for pedestrian traffic without a permanent passageway being appropriately marked thereby exposing employees to the potential of being struck by a fork truck.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: WAREHOUSE: On or about May 8, 2013, employees involved in cleaning the warehouse using granular cleaner containing sodium hypochlorite and sodium dichloroisocyanurate dihydrate were not required to wear chemical splash goggles to protect them from incidental splashes while mixing the solution and mopping the floors.

The employer had not developed and implemented a respiratory protection program with work-site specific procedures.
