29 CFR 1910.22(b)(2): Permanent aisles and passageways were not appropriately marked:  WAREHOUSE BETWEEN FREEZER AND COOLER: On or about February 13, 2013, an overhead curtain door located between the freezer and cooler intended for fork truck traffic was used for pedestrian traffic without a permanent passageway being appropriately marked thereby exposing employees to the potential of being struck by a fork truck.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  WAREHOUSE:  On or about May 8, 2013, employees involved in cleaning the warehouse using granular cleaner containing  sodium hypochlorite and sodium dichloroisocyanurate dihydrate were not required to wear chemical splash goggles to protect them from incidental splashes while mixing the solution and mopping the floors.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   WAREHOUSE:  On or about May 8, 2013, employees conducting deep cleaning in the warehouse were required to wear N-95 negative pressure particulate respirators.  The employer had not developed and implemented a respiratory protection program with work-site specific procedures.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  WAREHOUSE:  On or about February 13, 2013, temporary employees who were assigned tasks of cleaning in the warehouse were not provided training with respect to the chemicals they may come into contact with or be exposed to including, but not limited to: granular cleaner containing sodium hypochlorite and sodium dichloroisocyanurate dihydrate and spray cleaner containing ethanol and monoethanolamine.
