29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) At building 20, second floor outside ledge, on or about February 14, 2013, there was no fall protection for employees working atop the ledge. The top of the ledge was about 12 feet above the grassy ground below, thus exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder���s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. (a) Near the entrance to building 20, on or about February 14, 2013, a twelve foot stepladder used to access the second floor ledge did not extend at least 3 feet above the upper landing surface. The top of the ledge was about 12 feet above the grassy ground below.
