29 CFR 1926.251(a)(1):     Defective rigging equipment was not removed from service and rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe:  a) W 10th St & Shipley St, Wilmington, DE - Employees were using defective rigging (winch line), which had not been inspected prior to usage, to remove a light pole which weighs approximately 600 pounds. This violation was observed on or about February 13, 2013.
