29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  a) Intersection of Pennsylvania Avenue and Broom Street, Wilmington, Delaware 19806 - On or about 27 AUG 2018, the employer did not ensure that an inspection of the trench opened in the street was inspected before the start of work.  The proximity of the excavation to the surrounding vehicular traffic resulted in the formation of cracks and fissures in the trench walls.
