29 CFR 1910.1027(m)(4)(iii)(D): The employer did not ensure the employees were informed of the measures employees can take to protect themselves from exposure to cadmium, including modification of such habits as smoking and personal hygiene, and specific procedures that the employer had implemented to protect employees from exposure to cadmium such as appropriate work practices, emergency procedures and the provision of personal protective equipment: (a) Employees were exposed to cadmium when they worked with paint containing cadmium while weighing pigment and painting lamps and the employer did not ensure employees were informed of measures employees can take to protect themselves from exposure to cadmium, including modification of such habits as smoking and personal hygiene, and specific procedures that the employer had implemented to protect employees from exposure to cadmium such as appropriate work practices, emergency procedures and the provision of personal protective equipment. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets. (a) Acid Room - Employees were not provided training on hazardous chemicals in their work area at the time of initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Employees cleaned and removed paint from lamps in containers of hydrofluoric acid and their training did not include: the methods and observations that may be used to detect the presence or release of a hydrofluoric acid in the work area; the physical and health hazards of hydrofluoric acid in the work area; and, the measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hydrofluoric acids, such as appropriate work practices, emergency procedures, and personal protective equipment to be used; and, an explanation of the labels received on shipped containers and the workplace labeling system used by the employer; the safety data sheet and how employees can obtain and use appropriate hazard information. (b) Paint Booth - Employees were not provided training on hazardous chemicals in their work area at the time of initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. An employee painted lamps with paint containing turpentine, petroleum naphtha and naphthalene at the spray booth and was not provided training on the hazards of these chemicals at the time of initial assignment. (c) Soldering areas - Employees were not provided training on hazardous chemicals in their work area at the time of initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Employees using silver wire for soldering; employees using molten silver for soldering; and, employees using trichloroethylene and alcohol for cleaning lamps and other parts were not provided training on the hazards of these chemicals at the time of initial assignment. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (a) The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how labeling, maintenance of safety data sheets and employee information and training on hazardous chemicals in their work area will be met. Acid Room - Employees cleaned and reworked lamps in containers of hydrofluoric acid. Hydrofluoric acid is corrosive to the eyes, skin and respiratory system. It may cause lung congestion and inflammation. Stock Room, Paint Booth - Employees weighed pigment containing lead and cadmium in the Stock Room and painted lamps with paint containing lead, cadmium, turpentine, naphthalene and solvent naphtha at the paint booth. Lead effects the blood-forming, nervous, urinary and repoductive systems. Cadmium effects the kidneys, and may cause lung and prostate cancer. The paint solvent is flammable, irritating to the skin and eyes and may cause skin sensitization. Soldering areas - Employees use silver wire and molten silver for soldering. Employees use trichloroethylene and alcohol for cleaning lamps and other parts. Silver may cause skin, eyes and respiratory tract irritation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) On February 13, 2013, Advanced Strobe Products, Inc. did not record a work-related injury that resulted in medical treatment and lost work time on the OSHA Form 300 or equivalent for year 2013. An employee was injured when acid spilled on her leg on January 29, 2013. No abatement certification or documentation is required for this item.
