The employer did not assure energy control procedures were utilized to control hazardous energy when an employee(s) performed troubleshooting and/or repair activities to the machine's hopper car.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Reliable Castings Corporation - Sidney, Ohio: On or about February 5, 2013, the employer failed to assure that the Hunter mold machine was shutdown and procedures for the application of energy control were followed while an employee conducted service and repair tasks to the machine's hopper car. An employee repairing the car was exposed to injuries associated with pinch points when the hopper car unexpectedly moved to its home position during the repair.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Reliable Castings Corporation - Sidney, Ohio: On or about February 5, 2013, the employer failed to assure employees were provided with personal protective equipment to protect them from falls of approximately eight feet when accessing the top of the Hunter machine and the machine's board return. Reliable Castings Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1) and 1910.132(a), which was contained in OSHA inspection number 312213705, citation number 1, item numbers 1 and 4 and was affirmed as a final order on August 11, 2008, with respect to a workplace located at 3530 Spring Grove Ave. in Cincinnati, Ohio.
