Coastal dock, F/V U.S. Liberator, Factory: On or about 7 February 2013 and times prior thereto, a Tokai Header, Model PHC-550, machine number 159, 208V, 3PH; maintenance was being performed on the unit and it was not locked or tagged out. This condition exposed employees to a caught-in and amputation hazard.

29 CFR 1915.89(c)(7)(iv): The employer did not ensure the lockout/tags-plus coordinator maintained and administered a continuous log of each lockout and tags-plus system. Coastal Dock, F/V U.S. Liberator, Factory; on or about 7 February 2013 and times prior thereto, a Tokai Header, Model PHC-550, machine number 159 208V, 3PH; maintenance was being performed on the unit and it was not locked or tagged out. The lockout/tags plus coordinator did not maintain a continuous log of the lock/out tags-plus system. This condition exposed employees to a caught-in hazard.
