29 CFR 1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck: Forward Elevator, Starboard Side: On or about February 9, 2013 and at times prior thereto, a 6 X 26 inch unguarded area existed at the coaming which extended 22 inches above the weather deck. This condition exposed employees to a fall hazard of up to 60 feet.

29 CFR 1918.32(b): The edge of a hatch section, or of stowed cargo, was not guarded by a vertical safety net or other means providing equal fall protection to exposed employees: (a) Fish Meal Storage: On or about February 9, 2013 and at times prior thereto, fish meal bags were stored in tiers. This condition created a fall hazard of up to six (6) feet when accessing the top tier. (b) Freezer Hold, Elevator Platform, Forward: On or about February 9, 2013 and at times prior thereto, employees accessed an area of the eleveator platform when packaged product was displaced. This condition exposed employees to a fall hazard of up to 12 feet.
