Kloosterboer Dock, C/P Alaska Ocean, factory forward area; on or about 9 February 2013 and times prior thereto, the employer did not use a lock/tags-plus system before servicing the automated stuffing machine. This condition exposed employees to a caught in hazard.

This condition exposed employees to a caught in hazard.
