(a) Workplace:  The manufacturing floor and passageways where employees travel were not kept in a clean and orderly condition. Excess materials were placed within the passageways, exposing employees to a potential trip hazard, observed on or about 2/11/13.

B: Employees were using a Dayton Bench Grinder for grinding eye glass lens molds with the safety guard peripheral member at the top located three fourths of an inch (3/4) from the abrasive wheel periphery, observed on or about 2/11/13.
