Flowline Division of Ezeflow USA, Inc., 1400 New Butler Road, New Castle, PA, Production areas, on or about February 14th 2013, and times prior thereto: Company employees utilized liquid propane powered Hyster forklifts throughout their production areas. Hyster forklift (Serial # B010B05832J) was exhausting large amounts of Carbon Monoxide and needed mechanical repair. b.) Flowline Division of Ezeflow USA, Inc., 1400 New Butler Road, New Castle, PA, Production areas, on or about February 14th 2013, and times prior thereto: Company employees utilized liquid propane powered Hyster and Clark forklifts to move products throughout the production areas. Company employees were operating a propane powered forklift (Hyster brand; Serial #B177B020397) with no latch used to secure the propane cylinder. On some forklifts the propane cylinders were unlatched on this particular forklift the latch was broken off completely.

Flowline Division of Ezeflow USA, Inc., 1400 New Butler Road, New Castle, PA, on or about February 14th, 2013, and times prior thereto: Paint chips containing lead were present on the work benches in the Marking Deaprtment. Three bulk samples of paint were collected from the areas listed below and were analyzed to determine if lead was present. 1.) Sample 1(Flow-013)-Paint chip on cardboard side of the shelf in the marking area. 2.) Sample 2 (Flow-14)-Paint chip on the upper wall in the marking area. 3.) Sample 3 (Flow-015)-Paint chip on column 11 in the marking area. Analytical results for lead indicated the following: 1.) Sample 1(Flow-013)- 0.64 % 2.) Sample 2 (Flow-14)- 0.02 % 3.) Sample 3 (Flow-015)- 1.72%
