Flowline Division of EZeFlow USA Inc. - On or about 2/12/13, personnel entered confined spaces that included but were not limited to, the degreaser tank pit.  The company's confined space program stated either company employees would perform confined space rescues or an outside rescue service would be contacted to conduct the confined space rescue.  The outside service had not conducted an on-site evaluation of the company's confined spaces.

Flowline Division of EzeFlow USA Inc. - On or about 2/12/13 the mechanical power press did not have guarding or controls in place to keep employees out of the point of operation.  The Ajax press (1000 Ton; Serial #5055) was operated by just the foot pedal and its two hand controls were not utilized.       B.) Flowline Division of EzeFlow USA Inc. - On or about 2/12/13, the mechanical power press was not adequately guarded to prevent the operator and helper employee from getting any part of their bodies into the machinery and point of operation while it was operating.  The Ajax press (1000 ton) had two hand controls for the operator but no measures were in place on the other side of the press when two people were operating the equipment.     C.)  Flowline Division of EzeFlow USA Inc. - On or about 2/12/13, the mechanical power press was not adequately guarded to prevent the operator and helper employee from getting any part of their bodies into the machinery and point of operation while it was operating. The forklift did not have a new plate installed in denote the new capacity limitations:    A.)  Flowline Division of EzeFlow USA Inc. - On or about 2/12/13, employees were using a manbasket attachment on a forklift (Hyster H60XL; Serial #B177B02039L) to make structural repairs to the building.  The attachment had not been authorized/approved by the forklift manufacturer.  In addition, the plate had not been changed to reflect the capacity change with the attachment.
