The Ajax press (1000 ton) had two hand controls for the operator but no measures were in place on the other side of the press when two people were operating the equipment. C.) The forklift did not have a new plate installed in denote the new capacity limitations: A.) Flowline Division of EzeFlow USA Inc. - On or about 2/12/13, employees were using a manbasket attachment on a forklift (Hyster H60XL; Serial #B177B02039L) to make structural repairs to the building. The attachment had not been authorized/approved by the forklift manufacturer. In addition, the plate had not been changed to reflect the capacity change with the attachment.
