29 CFR 1918.11(a): The employer used the vessels cargo handling gear without ascertaining that the vessel had a current and valid cargo gear register and certificates in accordance with ILO 152 recommendations. Coastal Dock, F/V Blue Pacific; on or about 7 February 2013 and times prior thereto, a Slattery telescopic crane, Model MCT 1240TZ, unknown serial number, port mid-deck crane was used for off-loading operations, without ascertaining that the crane had a current annual inspection. Last certificate issued by Arxcis, Inc., expired on 04 August 2012.The condition exposed employees to a struck-by hazard.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace: Coastal Dock, F/V Blue Pacific; on or about 7 February 2013 and times prior thereto, the employer did not have material safety data sheets for all hazardous chemicals in the workplace. Material safety data sheets were not available for CRC Brakleen parts cleaner, CRC QD contact cleaner and WD-40 lubricant. The condition exposed employees to eye and skin hazards.
