OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    Engine Space:  On or about February 10, 2013 and at times prior thereto, three spaces on the backside of Generator #3 Controls were not effectively closed.  Space number one was unlabeled and measured two by one inches (2 X 1"), space number two was unlabeled and measured two by one inches (2 X 1"), space number three was labeled "SPARE," "P434," "4OAT," and measured three by two inches (3 X 2").  This condition exposed employees to electrical burn hazards from contact with the 480 volt, 900 amp bus bar.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:    Alaska Warrior:  On or about February 10, 2013 and at times prior thereto, the emplyer did not have a material safety data sheet for Liquichlor 12.5% Solution.  This condition restricted the information on the corrosive affects of the cleaner and  exposed employees to eye and skin hazards.

29 CFR 1910.1200(e)(1)(i): The written hazard communication program did not include a list of the hazardous chemicals known to be present, using an identity that was referenced on the appropriate material safety data sheet:    Alaska Warrior:  On or about February 10, 2013 and at times prior thereto, the last chemical list developed for the establishment was July 27, 1995.  This condition hindered the retrieval of material safety data sheets for hazardous chemical used in the workplace including the corrosive cleaner Liquichlor 12.5% solution.

29 CFR 1910.27(d)(4): Grab bars were not spaced by a continuation of the rung spacing when they were located in the horizontal position:    Factory Deck, Freezer Hatch:  On or about February 10, 2013 and at times prior thereto, the fixed ladder used to access the freezer hold did not have additional grab bars to assist in ladder use.  This condition exposed freezer crew workers to a fall hazard of up to 10 feet 4 inches.
