29 CFR 1915.89(c)(2):     The employer did not ensure the use of a tags-plus system to prevent energization or startup, or the release of hazardous energy, before any servicing was started.    Offshore Systems Inc. Dock, F/V Alaska Warrior, factory area; on or about 10 February 2013 and times prior thereto, the employer did not use a lockout/tags-plus system before servicing the Tokai Head Cutter Machines, Model PHC-550. Four of the five machines: #1- PHC-550L, #2 - PHC-550R, #4 - PHC-550R and #5 - PHC-550R were energized. Machine #3 - PHC-550L, was not tagged and was not energized. This condition exposed employees to a caught in hazard.
