29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 9, 2013 an employee operating the Haas TL-3 lathe, serial number 3091966, was exposed to struck-by hazards in that the lathe was allowed to spin at full speed with the doors open with the use of a manual override switch.
