(c)  Dock, Crane #3:  On or about February 6, 2013 and at times prior thereto, the Aurora Model Number 40 KNG 3000, Serial Number 91189-11791 pedestal crane was not equipped with a rating chart.  This condition exposed employees to a struck-by hazard in the event the crane was operated outside of its prescribed limits.
