29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a.)  East Coast Repair & Fabrication, LLC, Shipwright Street location, Electrical Shop, Dayton 6" bench grinder, Model 2Z425U, 1/4 hp, 3450 rpm, 3 amps, 115/230 volts, with a wire brush wheel on the left side, and an abrasive wheel on the right side that was used for sharpening tool bits did not have the right tongue guard properly adjusted in that it exceeded the 1/4 inch requirement. The tongue guard was adjusted to 5/8 inch creating a hazard to employees of being struck by debris should the abrasive wheel shatter during use.      b.)  East Coast Repair & Fabrication, LLC, Shipwright Street location, Inside and Outside Machine Shop, FCA 104 bench grinder, Model 114818, 220v, 3hp, 1250 rpm.  On or about January 15, 2013, the  bench grinder with a left side wire brush wheel, and right side abrasive wheel used for sharpening tool bits, did not have the right tongue guard properly adjusted in that it exceeded the 1/4 inch requirement.  The tongue guard was adjusted to 1/2 inch creating a hazard to employees of being struck by debris should the abrasive wheel shatter during use.

29 CFR 1915.134(b):     Work rests were not kept a distance not to exceed 1/8 inch from the surface of the wheel.    a.)  East Coast Repair & Fabrication, LLC, Shipwright Street Location.  On or about January 15, 2013, the Dayton 10" double abrasive wheel pedestal grinder, used to remove metal burrs and sharpen tool bits, did not have the work rests properly adjusted in that both of the work rests exceeded the 1/8" requirement.  Both work rests measured 3/8 inches from the rotating wheel creating an opening for the tool to become lodged resulting in an abrasion to employees' hand during grinding operations.
