29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): On or about February 12, 2013, employees were working on Rig #9. On the mud pit floor heading toward the rig floor stairway; there was an open space approximately 1foot wide where a guard was missing. The height measured approximately 9.5 feet to the ground below. Employees were exposed to potential fall hazards.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs: On or about February 12, 2013, employees were working on Rig #9. On the stairway leading up to mud pit on the eastside; the second stair from the top was damaged extensively which affected the riser height. This condition exposed employees to trip and/or fall hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about february 12, 2013, employees were working on Rig #9. In the generator room, Generator No. 1 had a pulley and belt system that was not guarded. Employees were exposed to potential caught-by and/or amputation hazards.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about February 12, 2013, the trailer where the tool pusher was staying was not grounded. Employees enter the trailer sporadically when called in by the tool pusher or when they need to speak to tool pusher in confidence; this exposed employees, including the tool pusher, to potential electrical hazards.
