29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:    On or about March 25, 2015, at the rig site, employees were exposed to fall hazards in excess of 6 ft. when the employer failed to ensure a top rail and midrail extended between the top of the staircase and the wall of the mixing room.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.      On or about March 25, 2015, at the rig site, employees were exposed to eye injuries when the employer failed to ensure the eyewash station in the mud mixing area contained water.       O'Ryan Drilling was previously cited for a violation of this occupational safety and health standard or its equivalent standard ,which was contained in OSHA inspection number 313619694 citation number 1 item number 3 and was affirmed as a final order on 11/08/2010, with respect to a workplace located at Ted Johnson Lease, Martin County Texas.
