29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement: a) Residences at Towamencin, Southwest side, Third Floor Patio - On or about February 13, 2013, a worker preparing to apply stucco to the patio wall while working on a tubular welded frame scaffold located approximately 12 inches from an unguarded edge of a balcony more than 16 feet above grade, and was not fully cross-braced to prevent displacement while under the load of material and/or personnel.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Residences at Towamencin, Southwest side, Third Floor Patio - On or about February 13, 2013, two workers applying stucco to the exterior of a building were working from tubular welded frame scaffolding that did not have a ladder extended to the top planked level of the scaffolding, requiring both workers to use the scaffold's crossbraces or frame end to reach the top planked level and then for descending to reach the ladder during egress.
