29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxillary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substanially equivalent to the wall of the equipment: On or about February 13, 2013, the circuit breaker box panel door assembly, located in the change house of NorAm Drilling Company Rig #24 was not screwed shut. This condition exposed the employees to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about February 13, 2013, the strain relief of the conductor under the circuit breaker box, located in the change house of NorAm Drilling Company Rig #24 was damaged. This condition exposed the employees to electrical hazards.
